     Report a Grievance or Complaint 

  

  • Purpose is to give the right to report a grievance or complaint and for conflict resolution. 
  • OHH will assist with resolution.
  •  A formal grievance must be filed within 20 working days following origin of the grievance. 
  •  When a formal grievance is filed, all the information requested on the form must be provided. The description of the grievance should include the names of other persons involved in the act, omission or occurrence.
  • The normal course of action in the grievance procedure is as follows:
  •       a) File with DON/administrator. All parties may consult with and receive the assistance of the DON/Administrator in resolving a grievance. 
  •        b) Maintain a personal copy. The grievant is responsible for maintaining copies of the documentation he or she provided for his or her records. 
  •       c) The Agency will document the receipt of the complaint/grievance and initiate a complaint investigation within 10 calendar days. 
  •       d)  The Agency will complete the investigation and documentation within 30 days after the receipt of the complaint, unless the Agency has and documents reasonable cause for the delay.
